Mental Health Care That Fits Your Life in Multnomah County
Finding a PMHNP you actually feel comfortable with is harder than it should be. Large clinic networks are impersonal. Wait times stretch for months. And when you finally get an appointment, you get fifteen minutes.
Thrive Psychological Wellness works differently. Caroline Thiongo PMHNP-BC runs a focused private practice in Portland and Multnomah, keeping caseloads manageable so each patient gets the time and attention their care actually requires. We work with adults and adolescents across a wide range of conditions including anxiety, depression, ADHD, bipolar disorder, addiction, sleep disorders, and more.
The approach is straightforward: listen first, diagnose carefully, and build a treatment plan that fits your life. Medication is one tool among many, and we never present it as the only path forward.

Common Questions About Psychiatric Care in Multnomah County
What is the difference between a PMHNP and a therapist?
A Psychiatric Mental Health Nurse Practitioner (PMHNP) is an advanced practice registered nurse with specialist psychiatric training who can diagnose conditions, prescribe medication, and manage psychiatric care. A therapist provides talk therapy but cannot prescribe. Many patients benefit from working with both, and we refer to therapists when that is part of the right plan.
Do I need a referral to see a PMHNP in Portland or Oregon?
Not always. Most Oregon insurance plans allow you to book directly with a PMHNP without a referral from your primary care provider. We recommend checking with your insurer beforehand, and we are happy to help you verify your benefits before your first appointment.
How long does it take to get an appointment?
We keep wait times as short as possible. Most new patients are seen within two to four weeks. Telehealth appointments are often available sooner.
Does insurance cover PMHNP visits in Oregon?
Yes. Most major Oregon insurance plans cover psychiatric nurse practitioner care. Oregon Health Plan (Medicaid) also covers PMHNP services. We verify your coverage before your first appointment so you know what to expect.
Do you offer telehealth appointments?
Yes. Thrive Psychological Wellness offers secure, HIPAA-compliant virtual appointments for all new and existing patients anywhere in Oregon. All you need is a smartphone, tablet, or computer with a camera.
What conditions do you treat?
We treat anxiety, depression, ADHD, bipolar disorder, OCD, PTSD, substance use disorders, sleep disorders, emotional disturbance, family conflict, and more. Visit our services page for the full list.
Can a PMHNP prescribe medication on the first visit?
Sometimes, but it depends on the evaluation. The first appointment is a full psychiatric assessment. If medication is appropriate and the clinical picture is clear, a prescription may be provided at that visit. We never prescribe without a thorough evaluation first.
Do you see adolescents as well as adults?
Yes. Caroline Thiongo PMHNP-BC sees adolescents and adults for most conditions including ADHD, anxiety, depression, bipolar disorder, ODD, and mood disorders.
